As a way of saying thanks for being our patient and to encourage patient involvement in their orthodontic treatment, we have created “The Ortho Bucks Program.” This program is designed to reward our patients who successfully comply with their orthodontic care by giving them Ortho Bucks that can be redeemed for many great prizes.

Prizes

There are many prizes to choose from to satisfy our patients of all ages. The following are just a few of the many prizes available:

1. Portable CD Player
2. Gift Certificates
3. Alizadeh Shirt
4. Game Boy Games
5. Purchase lost, broken, or replacement appliances

Ortho Bucks Program

Patients have the opportunity of collecting up to five ortho bucks at each regularly scheduled appointment. Ortho Bucks may be saved and redeemed for a variety of prizes. This is our way of saying “THANK YOU FOR BEING A SUPER PATIENT!”

How to Earn Ortho Bucks

You will receive 5 ortho bucks, when you start your orthodontic treatment. Below is a list of how you can earn ortho bucks at each regularly scheduled appointment.

1. On time for appointment 1 buck
2. Good Oral Hygiene 1 buck
3. Nothing broken since last regular appointment 1 buck
4. Wear your Alizadeh T-Shirt 1 buck
5. Schedule next appointment 1 buck

Ways to Earn Bonus Ortho Bucks

There will also be ways during your treatment to earn bonus ortho bucks.

1. Refer your friends to our office for a consultation 5 bucks
2. Visit your dentist for your regular 6 month check-up 5 bucks
3. Bring retainers to appointment 1 bucks
4. Monthly appointment slip drawing 10 bucks

How to Redeem Ortho Bucks

At the end of each regularly scheduled appointment take the slip that your assistant gives you showing total bucks earned to the front desk to pick up your Ortho Bucks. You may cash in at any time or save as many as you like; there is no expiration date.

RULES

1. Ortho Bucks are awarded at your regular appointments. Walk ins or emergencies do not apply.

2. The number of Ortho Bucks you earn is up to you.

3. A prize board will be up with current prizes available. They may change from time to time. Not all prizes will always be available.

4. Your Ortho Bucks are your responsibility. They are just like real money; if they are lost they are gone.

5. Once you spend your Ortho Bucks, you must earn more to purchase more prizes.

6. The computer will be the judge of an “on-time” appointment. Make sure it accepts your name.

7. Your assistant will be the judge of good oral hygiene.

8. You must bring a signed form from your regular dentist showing that you kept your 6 month check up.

9. If you refer someone to our office be sure they mention your name.

10. Once you are dismissed as an active patient you can no longer participate in the Ortho Bucks program.

11. Any are all broken brackets for any reason are not eligible for Ortho Bucks
